This metal bracelet with yarn counter slips over the knitters wrist, hooks to a bag, belt loop, or a table.  A second piece of metal slips into the center of a skein of yarn and then is hooked to the bracelet.  The yarn is pulled from the outside, but the bracelet attachment swivels and keeps the yarn tangle free.
